Care, Meals and Home, Together
Every home has caregivers on duty around the clock, home cooked meals, and visiting physicians and beauticians who come to the house. Care is planned around each person and changes as they do.
Each of our three homes is a licensed Residential Care Facility for the Elderly (RCFE) providing a home-like setting for between six and fifteen residents at any given time.

What We Provide Daily:
- 24-hour care and supervision by trained, experienced caregivers
- Assistance with personal hygiene and the activities of daily living
- Mobility and transferring support, bathing, continence and dietary management
- Medication oversight
- Home-cooked, nutritious meals with dietary considerations
- Housekeeping and laundry services
- Well-furnished private and shared rooms and outdoor space at every home
- Daily exercises and activities, hair and nail services
- Medical coordination, including home health nursing and visiting physicians

Individually Planned
Each resident's needs are different. Care plans are tailored to the person and adjusted as those needs change, rather than fitting everyone to one schedule.
Hospice Experience
Our caregivers are trained by a hospice nurse to notice small changes early and act on them.
Genuinely Small
Six to fifteen residents per home means caregivers know each person's routine, history and preferences, not just their care sheet.
Staying Connected
Electronic documentation tracks health monitoring alerts and medication management, and supports clear, timely communication between staff and family members. You should never have to wonder how your loved one's week has gone.
Real Homes, Real Care
Older adults aged 60 and over, in real homes with outdoor space, private and shared bedrooms, and around-the-clock staff
Activity Programming
Activities include crafts, exercising, music and other projects, led by professionals from the surrounding community. Special visitors provide entertainment regularly, and families are always welcome to join. We accept private pay and long-term care insurance.
Questions Families Ask Us
We know choosing senior care brings up a lot of concerns. Here are the questions we hear most often:
How many residents live in each home?
Nurturehood in Concord, 6; The Villa in Auburn, 15; The Gardens in Auburn, 10. Each is licensed as a Residential Care Facility for the Elderly. Those caps are the whole point. They are what make it possible for caregivers to know each resident properly.
Who owns and runs the homes?
All three are owned and operated by a Hospice and Palliative Care Registered Nurse with 9 years of experience working with the elderly population. That matters most as needs change: shifts in appetite, sleep, agitation or comfort get noticed early and acted on.
What kinds of care do you provide?
Assisted living: help with personal hygiene, bathing, mobility and transferring, continence and dietary management, and medication oversight, alongside meals, housekeeping and daily activities. We coordinate with home health nursing and visiting physicians as needs change.
Which home is right for my parent?
It depends on where you are and what they need. The Villa in Auburn is our largest and most social; The Gardens, also in Auburn, is set up for residents who need a higher level of daily support; Nurturehood in Concord is our smallest, with a patio door in every room. Call us and we will say honestly which fits.
How do you handle meals and dietary needs?
Meals are home-cooked in the house using fresh, seasonal, local ingredients, with full table service and special dietary accommodations.
What does it cost, and what payment do you accept?
We accept private pay and long-term care insurance. Actual monthly cost depends on the level of care needed and the room selected, and is confirmed in writing before move-in. Call (925) 849-6208 and we will talk you through it honestly.
Can family visit whenever they like?
Yes, and the outdoor space is designed for exactly that. Visiting pets are welcome too, provided they are vaccinated and leashed. Resident pets are not permitted.
How will I know how my loved one is doing?
We use electronic documentation covering health monitoring alerts and medication management, and we communicate with families consistently rather than only when something goes wrong.
What happens as my loved one's needs increase?
Care plans are reviewed and adjusted. Because the homes are run by a hospice and palliative care nurse, residents may remain through the end of life with hospice services coordinated in the home, rather than being moved at the hardest possible time.
